No, I think not. Adding it here: C: \ Program Files (x86) \ Steam \ SteamApps \ common \ Team Fortress 2 \ tf \ cfg. What do you mean by putting autoexec set launch options.
You have to put it in SteamApps\common\Team Fortress 2\tf\custom\customfoldername\cfg
If you don't have the last 2 folders, just make them. customfoldername can be whatever you want to call it.

What class did you switch to? Maybe there's something in that classes cfg that changes mouse2.
You could always type "exec medic" in console.
edit: actually, just put bind mouse2 +attack2 in medic.cfg
