Location of the folder doesnt matter afaik, I have NvidiaInspector folder just on my desktop and its fine.
Set "Antialiasing - Transparency supersampling" to AA_MODE_REPLAY_ALL

and apply changes. Then export the TF2 profile by clicking this icon

and then "export current profile", name or location of the file doesnt matter.
Then open the exported profile in some text editing program(notepad is fine) and ctrl+f

what you're going to edit is the value of this setting, this is quote from ETF2L thread http://etf2l.org/forum/customise/topic-17728/
to achieve a LOD Bias > 3, you need export the “profile” and change the Setting (in the exported Profile):
SettingID:7573135
SettingValue: YOURDESIREDVALUE * 8
LODBias: 10 you would enter 80
for 15 enter 120.
so what you want to do is change the value to 120 and save it in that exported profile file.
Then you go back to NvidiaInspector and import the profile by clicking this icon

and then "Import profile(s)", after this you should see that "Texture filtering - LOD Bias (DX)" has changed itself to 0x00000078

then click apply changes and you're free to close NvidiaInspector, now when you start TF2 it should have lower LOD.
