I believe its the HudClassImage.res for the class image, not sure about person healing you, but maybe its TargetID.res
Try deleting/editing those
EDIT:
Okay so for HudClassImage.res and TargetID.res (in resource/ui) just delete them and add the ones from broesel hud. In the hudlayout.res (scripts), find CMainTargetID and change the "tall" value to 56.
It looks fine for me afterwards so yeah. I'm in 1280x720, So I'm not sure if that has an effect
ANOTHER EDIT:
Again in hudlayout.res, go ctrl+f for SpectatorTargetID, change the tall value to 35 (you get a tiny little blue bar under it and can see the Ubercharge on meds if you spectate them) if you don't want the blue bar under the ID i believe its 32

tnx for your help but all of this is fine. the last thing i wanna know is how to change the font size like the video u linked. the font size of the hud that is in the video is huge but the normal one is small

Yes thats why, and hes using Highquality config for video I believe or Max. It looks really clean I like.
