Filled it in, but I honestly don't think these questions are thought out properly.
Talk to your prof and read up on how to set up questions that give you proper data to use.

It's an interesting idea and the questions are quite good but I agree that collecting data from them could be confusing or impossible

As the previous 2 have stated, you would have to read through every single one of these in order to get any sort of data and even then it would be so grey area at that point. I would totally be down to fill in a similar survey with a multiple choice format though, I like these sort of survey threads.
