Obviously situations in an actual game are different to those you find in dm and depend on context and gamesense to a greater/lesser extent to hit players but there is a certain amount of raw skill necessary to hit more difficult/consistent shots that is definitely hugely important to playing demoman at a high level. Also, the situations that you tend to practise in dm are generally some of the most mechanically demanding of all of those in a 6s game (e.g. getting rushed by a scout/ bombed by a soldier) which is why they need to be practised over and over.
