Uploading a fake log isn't very hard. It's a simple text file so you can just open it in notepad, ctrl+f and remove/change whatever you want. In this case the guy did that to remove all your entries in the log of damage/kills and changed the names of the players.
With some basic server knowledge you can then reupload it to logs.tf as a new log

It keeps recording after the game though you can just use "stop" in console to end it or leave the server
You can bind the in-game recording to be manual. Once the countdown begins you just press the button. That way pre-game isn't recorded
Rarely you'll realise you forgot to press record after you make a good play but there's always STV
edit: bind f7 ds_record;bind f8 ds_mark
Pretty sure those commands are correct. Record starts the demo, Mark is for bookmarks. try "find ds_" in console for finding the other commands like minimum killstreak/killstreak delay/delete if no bookmarks/etc

probably a flaw in the vmt files. Make sure the vtf has a corresponding vmt file and open the vmt in notepad to make sure the correct vtf name is also used inside
Cowmangler doesn't make enough of a difference compared to stock to justify so many people complaining about it
