Try running the game with "-autoconfig" in the launch options. If issues persists, it might be something other than your config/game.
Additionally, you could also try and "verify the integrity of the game cache" in Steam by going to your games library, right clicking on TF2 > Properties > Local Files > Verify Integrity of the Game Cache".

Good earplugs/IEM for gaming

The Shure SE215's are fantastic. The cables are also replaceable which means if they break, you don't have to shell out a ton of money for a new pair.
Amazon often has sales on them so depending on where you are you can probably get a really good deal.
