Getting very slow download speeds on Youtube (Twitch is fine). Can't even watch 240p most times.
It might be AT&T. A lot of people who have problems have U-verse internet (which I have).

I also have AT&T, kinda sucks that this problem exists.
So I can seem to play some videos just fine on 1080p, but others will barely buffer or not buffer at all on 240p.

Yeah this just started happening to me a few days ago. Every few months I get it for about a month. Fun times.

You should keep in mind that Google does global load balancing on YouTube videos; popular videos are stored on CDNs which are ridiculously fast, while less popular videos get pushed onto collocation networks that will load slower for everyone. In any case I doubt there's much you can do about it.

Don't think that's it. Just tested Gangnam Style (yes, I suffered through 5 seconds of it to test this out) and it loaded slowly. Still #1 on the Youtube 100 so I think it's really just random.
Maybe I can use this as an excuse to get my parents to switch to Comcast's 20/4 plan. Last I checked it was cheaper/the same price as what they're paying now for the first 12 months.

Saying the popular videos should always be blazing fast is a bit of an oversimplification. Google itself operates an extremely complex CDN that peers with local ISPs and nodes. Some videos get cached to nodes near you, others don't.
Additionally, there is always the chance that your ISP is doing shady shit. I remember this reddit thread that came up where someone found their requests were going to a third party CDN:
