Maps that aren't mirrored for both sides can make a map pretty imbalanced if you're not careful. Gravelpit is an example of a non-mirrored map that is balanced.
Also if there are just way too many chokes and way too many entrances/exits/flanks (look at Follower), the map won't play that well.

A 5cp wont be imbalanced as such unless it isnt properly mirrored (like granary) but what makes a map bad is usually choke points and height advantages which either make it too easy or too difficult to push.

Props. As a roamer some maps offer a ton of extra hiding spots to abuse on a certain side.

Class balance is important too. If the entire map is a hallway then the scouts are not gonna have a good time. If the map is too open with a shitload of props for scouts to jump around on then the soldiers are going to have to spend the whole game going for frag movie level directs on double jumpin' white guys.

Don't spam props to fill a room or make the map look nicer. They should have a purpose. Nobody likes getting stuck on weirdly placed props
