So I have a question

Is a turbo Twitch.tv account necessary to watch streams in any quality above the lowest, or are twitch's servers just not capable of handling all of the connections during prime time stream viewing times?

You can only switch stream quality if the channel is partnered. If you're watching someone that isn't partnered you have to watch at whatever quality the person streaming has it set at.

yah and almost all the time I try watching a stream at 480p+, the stream is usually extremely laggy. I thought it might just be me but a lot of people I know say that it's often laggy for them as well. I'm assuming 12mbdown and 1mbup is enough to watch streams at 720+ as well
