Same thing is happening to me with Quake Live and QuakeWorld. Maybe its an OpenGL issue?
Edit: Having updated to 23.4.1 all my fps issues have disappeared.

My goal to upgrade to get a 240hz monitor and a cpu able to maintain 240fps in 6s for less than $500. GPU is RX580 but i doubt tf2 would need more than that.
