I went through the same hellish experience about 2-3 months ago. Let me tell you I know exactly how frustrated you are. The solution to mine ended up being my cable modem and router were dying. I'd had the modem for about 5-6 years and the router was roughly 3 years old. Long story short I bitched at my ISP and told them my modem was dropping connection and they gave me a new cable modem (comes free from most ISPs as far as I can tell). Then I invested in a new router, and presto- No more ridiculously unexplained lag.
So my advice would be to try your computer plugged directly into the cable modem if it isn't already. Doing that will help you rule out if the problem is specific to your router or not. If the direct connection from your computer to the modem still sucks, and you're sure it's nothing about your computer, then I suggest you find a way to try another cable modem.

As far as midfights go, you need to be aware of the typical strategies people run depending on the map. If you know where the scouts or demoman usually come from, then you should anticipate where you can lay down your first stickies at the midfight. Prediction is a big part of playing smart demoman.
Also as far as looking at this question from a team perspective, work with your scouts and have them call where the important enemies are at the beginning of the midfight. Working with your scouts to control mid asap gives you an edge over your opponent. TF2 is a communication game. The better you can communicate with your team the more room your team has to grow.
