I'd say it's something neurological if you have problems peeing outside home and maybe bad blood circulation when you feel that mini blackout.
My advice is that you have to start drinking more water and consuming less sugars and carbohydrates even more if you're a sedentary person (maybe not your case cuz u go to the gym, but try this).

ok well, im not a doctor or anything but here is all i can say... when you pee at home are you standing up from taking a shit and then pissing, because the fact that you are standing up might make you feel woozy and give you a headrush and make it seem like you are slightly losing consciousness, for the thing at the gym, maybe its just anxiety.

Do you have the same reaction upon orgasm? How about when trying to push out feces? It is probably a circulation issue, and may be caused by trying to push out the urine too hard, vagaling down and causing a drop in your blood pressure. Alternatively, the rush of blood into and around the urinary bladder at the moment of release can cause this, but would be rather unusual. To fix, get in better shape, for some reason it is harder to bottom out blood pressure manually like that when you are in better shape. Troll post or not, I'm going to give a serious answer to this. The answer to the question depends entirely upon what you actually mean when you say 'my eyes go black'. Do your pupils just become instantly dilated, or are you indicating that you have a temporary loss of vision?
There are many things that can cause something like this to occur. Kidney disease often leads to swelling of the eyes, which may cause something like this. It's unlikely that this would be your first symptom since you'd be well-into the progression of the disease.
There is also a neurological disorder of the eyes called Adies Syndrome. This is unlikely as well, since the typical effect is that your pupils will stop responding to light and will essentially stay dilated all the time--but it's worth mentioning because its early stages have symptoms similar to yours.
Far more likely is something like vasovagal syncope/neurocardiogenic syncope--a sudden loss of heart-rate and blood pressure. This is difficult to diagnose since you have to rule out a dozen heart problems first.
