I used to be named AzzyTheGamer, I got this name from a book a read and the main characters name was Azoth. (great book btw) Then I was Azzy, same reason.
Finally I reached MrPerfect. No real reason behind it other than Im just show how I am way better than Noona :)

I was roughly 9 or 10 and watching teen titans at my cousin's house, it was 10 o'clock at night which was a big deal for us and this episode was running while we were playing crappy dragonfable:

My cousin made a rogue and he ran with a name that was very close to his name (kfcmatt, he is quite the sucker for the fried chicken) and it was my turn to make a character- and me being 10 years old- and I was stuck with a name, because for some reason this was a life changing decision for me. So I was stuck on the character creation screen and I didn't know what to do, so I just kept watching teen titans. Eventually in the episode, Killer Moth yells his name out, and we thought it was funny so we started exclaiming Killer Moth into the air in a bunch of really dumb voices, and soon our words blended into "Killamoth." We thought this was even funnier than the Killer Moth thing, so we kept going and dying of laughter until he told me that I should make Killamoth the name I'd use. I was hooked. So I made my warrior with the name Killamoth, and the name stuck for a long time. And now I use Prandonb which is a half-spoonerism of Brandon P which is just my first name + my last initial.

Back when I was 13 years old, there was a class on "How to Use the Internet" at our middle school. One of our assignments was to sign up for an email account. My favorite car at the time was the Plymouth Roadrunner.

When I made my first AOL account (with my parents hovering over my shoulder), I made it the nickname my sister called me (for being weird) oddjobbob, with 705 tacked to the end as my birthday.
I used that for about 5-6 years, until I changed it to Bobbeh (Bobby with an EH instead, god i'm clever) on this Naruto forum I used to frequent.
I got Xbox Live about 1-2 years later, and made it Bobbeh705, since Bobbeh was already taken. After about a year of that, I got pretty bored of the name, so wanting something creative, I decided to go as 'effeminate' as I possibly could, and ended up with Unic0rnSunshine.

The character is Phil Ken Sebben, a parody of the Falcon Seven character from the original Birdman cartoons, who was basically Birdman's boss, and is now his boss at a law firm. He's voiced by Stephen Colbert. The show is funny as hell if you watched any of these old cartoons, and even if you didn't.

Back when I was in my sophomore year of high school my friends and I were hanging around in front of our school before class started. 3 of them were talking about a band they liked called Five Iron Frenzy. They were discussing one of the songs that dealt with growing older, and one of the lines in the song was something like "we'll hit the boardwalk just like Matlock".
About a second after he said it, my friend just slowly turns to me with his eyes wide open and a goofy grin on his face, points to me and says "you're Matlock now". I had no idea who Matlock was at the time, so I asked them who the hell he was. They explained that it was the name of a TV show that old people in retirement homes liked. So the next thing out of my mouth was an irate "not Matlock".
From then on they kept calling me Matlock, to which I would always respond with "NOT MATLOCK!" Eventually in my senior year, one of our friends came to school with an old disk with America's Army 2 on it. Over the next few days we traded it off to each other so we could install it on our computers, bring it back to school, and give it to someone else to install.
Eventually when we all had it installed we would join up and play together and so we could communicate we would set up a conference call and talk to each other that way. My original name in the game was Bone (a nickname I picked up in elementary school). Unfortunately, everyone kept calling me Matlock, so out of frustration I changed my name to Not_Matlock.
The name stuck ever since then. When the Xbox 360 came out and I bought one, I set up my name as Not Matlock (Microsoft doesn't like underscores apparently) because everything else that I could think of was already taken and I didn't want to add a bunch of useless numbers or letters to my name When I bought HL2 a few months later I used the same name out of consistency's sake.
