I always assumed tip of the hats would be a different charity each time (I don't know why), but I'm fine with sending more kids to camp. I think it should be an all class melee weapon that's a stick with toasted marshmallows or hot dogs on the end (maybe a smoke particle effect?). Other than that the water wings are also a good idea.

Not trying to be insensitive here, but I know the hair loss associated with treatment can be an issue for cancer patients.

Often you here stories of friends and families shaving their own heads in solidarity.

"Shaved Head of Solidarity" might be one simple hat idea?

I know the Heavy is already bald and the Engineer has this Texas Slim's Dome Shine etc., but I think it could be a more poignant symbol that makes people stop and think?

with as many camps as there are, even if it is a different charity/focus it can still "send kids to camp" no matter what the group...

I like that idea a lot, but if it was anything but a reskin it would need to be considered by Valve as an actual weapon addition to the game, and that seems like more time would be involved in getting items into the game.
