Sometimes it spikes when I rj as well. I've heard of this before but I can't remember if there's a solution or not.
I've been having a similar issue but it seems to go away sometimes.
The weird thing is my upload and download rate are completely normal for my internet package.
I'll report back if I find a solution or tweak but it's most likely on yours/my ISP's end.
I was having similar issues, everything seemed fine in some servers and then in others I'd have brutal lag spikes as well as lag whenever I used my mic.
I seem to have fixed it by turning off UPnP on my router, hopefully that helps someone (maybe not OP as you've checked multiple routers).
I have a relatively obscure isp by the name of T6 Broadband. Their umbrella company is JAB Wireless.

Talked to my ISP, talked on dslreports.com, and while I've found some people with similar problems I still haven't gotten anything resembling a solution.
I've been having this issue since roughly November of last year, and even before that it was off and on from January 2011. I spent months and months calling my ISP, getting techs out, emailing them tracerts and all that jazz. Never got anything but lies and a bunch of bogus stuff. "Yeah, should be better in a month or so, we're upgrading our hardware." "If the problem keeps happening please let us know, but we can't do anything about it at the moment." It just got to a point where I knew I wasn't going to make any more headway and decided to give up. They're the only option of an ISP I've got atm, I live in a pretty rural area, and moving is not an option just yet.
I know this isn't going to help you or even make you feel better about the situation, but I guess I just wanted to let you know that you're not alone in this, for what it's worth. Keep on fighting the good fight, maybe you'll have better luck than me.
