Sometimes never, but the chance of a false positive is super low.
One of my friends got VAC'd and the dude gave me complete access to his PC, after 2 days of checking everything he has installed and looking into all of his windows ShadowCopies to see if I can find LMAOBOX I found 0 proof that he used any cheat application. I emailed all this to valve and they never responded, my buddy is vac'd to this day and lost a 2k backpack.

Yes miggy, it would be impossible to erase all evidence prior to granting you access to his computer. I don't understand why valve wouldn't take your word on it.

ShadowCopy let's me view backup copies of folders on his PC, Using this I was able to view the contents of his user profile on his PC and check over various folders in his Root of C:
Not only that but depending on how often your automatic backups run it can keep shadowcopies going back days or even weeks.
So, unless he is a precog and foresaw that he would get vac'd and lose access to his items but decided to only delete the hack and not save his items then I don't get your argument.

The guy doesn't play competitive and he coveted his items too much to lead to believe that he would ever cheat, let alone on his main. He uses alt accounts to help populate his servers in the mornings, if he wanted to cheat to pwn pub noobz he would have done it on those.

what even triggers a false positive? i know that SAM has been rumored to cause VAC bans, but i can't think of anything else.

It is impossible for SAM to trigger VAC as it does not interact with the tf2 executable at all. Anyone claiming that it does is simply using it as a scapegoat.

Someone else in EU who definitely wouldn't cheat got VAC banned for no reason yesterday. Could they be connected?

Majesty (brazilian TF2 player) got a false-positive in CSGO last year. He e-mailed the guy responsable for VAC in Valve (Vitaliy) who then answered he was going to check his case. Like 2 days later his account was unbanned. He sent another e-mail to thank Vitaliy who answered he didn't do anything, the system removed the ban automatically. All of this happened in like 1 week.

What people need to understand is that VAC bans are only triggered when people cheat on VAC secured servers. You could have all cheats known to man on your desktop, you could be hacking into the white house, you could even be cheating on non-vac servers but none of that will trigger a VAC ban.

Yes you need to be on a VAC Secured server when the "detection" happens but a couple things could cause you to get flagged
Running an unapproved clientside plugin could trigger it
A server side mod that changed render settings caused a wave of vacs at one point
Malware that modifies your system files
-Cryptolocker has caused this to happen to some users, either way they were fucked

There's a lot of stuff that could trigger it but it's really hard to determine as Valve, understandably, keeps the details of their system close to their chest.

There's plenty of stuff that's not exactly a cheat and still triggers VAC. If it injects something into the client it's always risky.

For example HLAE uses this method and is considered a cheat by VAC, but as long as you launch game in insecure mode and dont join any vac servers you're good to go.

But for example I have binds to join servers in my config, if I accidently launched tf2 in secure mod and pressed one of those binds by accident while recording it would be pretty unfortunate but probably would get a vac ban while not cheating.

That'd be true except insecure plugins are not enabled when tf2 is launched in secure mode, that's the whole point of secure mode and why it even exists.
