These craters fucking annoy me whenever I see them and can be really distracting sometimes. I don't know when it happens but usually whenever there's a lot of rocket spam. Sometimes they appear sometimes they don't. I thought it was a decal but it didn't go away after I did r_drawdecal 0

Never seen that. Must be pretty rare. Decal is the first things that comes to mind but seems like you have already checked that

i've had these before, iirc i was using the decal removal thing in clean tf2 and when i removed it they disappeared. i could be completely wrong but if you're using that try it.
