when i fall into a super-deep youtube hole i tend to watch old uscsb accident videos, enough for my friends and i to develop a drinking game based on how predictable they are

Drink if:

established standard procedure was skipped or ignored for the sake of efficiency
employees were undertrained or were never trained at all, leading them to make otherwise innocent decisions that kill actually everyone
rescue / damage control efforts devolve into slapstick comedy of mistakes and bad timing
