Don't be afraid to hold W.

This is actually an extremely loaded answer, but it's true. It is quite often linked with your confidence in your ability (see: clockwork, sheep, snowblind,elmo, others). While you don't want to think that you can fight the world, you want to go into the mindset that you quite potentially have the ability to win ANY duel you take. If you get into a duel and just go "I'm on 2 hp there's no way I'm gonna win", then you'll probably never clutch a situation similar to that, (EDIT:)especially if it's an important moment in a game.

My recommendation is to just study some demos, mostly of transitional/phase play to see how both scouts on each team react to what their team does and the other team does. Rollouts are cool and all but they are 15% of the game.

highlander is a community-developed gamemode that removes class switching entirely from the game and arbitrarily enforces the all-classes-are-equal mantra which the developers (to my face) have said was never intended

Maybe so, but they certainly didn't intend for the same 4 classes to be used 90% of the time and a significant amount of content arbitrarily banned.

I think, however, most of the stuff from 6v6 is closer to Valve's image of the game competitively.

I remember one of the first MMO's I played was this Korean(?) MMO called Dragon Raja. It was kinda cool but all I did was just level up a few times and not really pay much attention to anything (p much standard for most MMO's these days, eh?)

Another funny memory (not the start of my online gaming, but early enough) was back in '04 in Grade 7 (first year after Elementary School for you yanks etc). I remember playing some fy_iceworld pub in CS 1.6 and I saw a guy's real name that attended my school. I dumpstered the server (heh) and the next day at school he was pretty fucking mad and had a go at me.
