This doesn't look too good...
By the look of the injured + blood, I think someone/some people lost a leg...

TurinThey actually do secure the whole 26.2 mile length and have security cameras along the way, and police stations. They do this due to the huge amount of spectators, and along with preparing for normal mob activities, they also do some counter terrorism according to the Boston Police. They should expect this.
Unless they strip search everyone, it's entirely possible that someone snuck the bomb in backpack. It's extremely hard to keep track of someone who does that, let alone identify that. I honestly think they'd have to have gone full TSA to have caught that. I think the only thing they can do is prepare/react to any possible aftermath as best as possible.

The explosions looked a lot smaller than expected and they almost seemed like home-made. The current outcome is so much better than what we would expect from an attack on large group like this. Hope the people injured would have speedy recoveries and keep that death count at 2.

Even then it'd of been nigh-on impossible to catch this one, if it wasn't a suicide bombing the list of suspects jumps pretty quickly up into the hundreds.

Police said they couldn't give an estimate of the number of injuries and it's important not to spread rumours at this point.

In other news, 31 people were killed by bombs today in Iraq, http://www.bbc.co.uk/news/world-middle-east-22149863
Edit:
Just to be clear, I don't think the prayers for Boston are unjustified nor people being worried, I do hope anyone affected by this event only hears good news about its results. I'm merely showing how similar events happen daily in other countries.

#57 If someone untrained was to handle something like this they could make things worse. Its best to leave events of horror to the men and women trained to help in times of need. Also they didn't know how many more devices there were in the area so people were told and decided to get away from the chaos. It looks bad but it seems like everyone was doing their best.

clamwhy do people stop to take pictures and video instead of help? i understand it needs to be documented, but why to this point?
When people suffer serious injuries, it can be more harmful than helpful for someone who doesn't know how to handle the injured to try to intervene.
