I had an excruciating double ear infection, one that I could feel even through extra strength advil. Also my dad was diagnosed with brain cancer when I was 8 and died 3 years later, after we thought he was tumor free twice.
I remember when I was seven I let a magazine salesman come into our house and he took a really bloody dump in our toilet. I was up watching Saturday morning cartoons when he arrived at the door, my parents were still asleep. I told him my parents weren't home, only because I didn't want to have to wake them up and miss my cartoons. He then told me that he was a good friend of my dads, so I let him into the house and went back to watching TV. I didn't pay much attention to him after that, short of hearing an enormously loud fart coming from the bathroom. By the time my parents got up he was gone. My dad went to the bathroom that morning and screamed when he saw the gigantic bloody turd in there. I remember it vividly. It looked as if someone had taken a pint of bloody mary and just tossed it right in there. The guy must have had colon cancer or something. I really just don't understand how there was that much blood. Anyway, my dad's bass guitar was also missing and my dad beat the living shit out of me with a set of jumper cables for letting the guy in. But to this day I often find myself thinking about that bloody toilet, and the magazine salesman who I'm guessing is no longer alive, and the thought of it scares the crap out of me.
Lumbar puncture. I've had a lot of cuts and sprains, a few broken bones and even kidney stones in the past but nothing even comes close. It was so painful I didn't even recognise the sensation as pain at first. Super weird and intensely agonising. It was a junior doctor as well so they were rooting around for quite a while, kept scraping the shit out of the periosteum with the needle.
i had a needle shoved up my dickhole without painkillers when i was a young boy
horseshoe hit me straight in the fucking big toe, fucking broke it. worst part was the nurse wrapping my big toe and 2nd toe together, shit hurt like a mother fucker.
Sciatica for 5 months was pretty bad, I basically couldn't leave my bed the majority of the time due to the pain. At its worst lying down was a solid 7/10 on pain, sitting was an 8, and I couldn't stand for more than 5 or so minutes without breaking out into a cold sweat and shakes, with pain increasing steadily.
Worst pain was when I got a steroid injection in December for it, and the doc only used local anesthesia. For those who don't know, steroid injections are meant to reduce inflammation at the site where a hernia is pressing on the sciatic nerve. However, it initially (for a few hours) adds volume to the pressure since you're injection a liquid into the site. Anesthesia didn't work, so it felt like I was being cut in half.

I've been lucky with physical pain too, never broke a bone or anything, I think the only time when I truly felt deep pain was with appendicitis

Is there anything you did besides the injection that helped?

In the first 3 months I tried physical therapy, epidural injections, heat therapy, acupuncture, daily stretches (turns out they're basically kegels lmao), basically every non surgical option available. The only things I didn't do were chiropractors (don't trust them) and anti-inflammatory oral meds. I had 3 injections total spread out over a period of 1.5 months, which helped a little bit after the third one. The first two helped for like a week or two, but nothing long lasting. Nothing else really helped except narcotics (percocet and vicodin) and the surgery at the end (microdiscectomy).
late edit: I forgot that I also tried weed after the first injection. Felt awesome with no pain when I was high, incredibly painful right after. It was like the brief period of no pain made me notice all the pain that I was ignoring subconsciously, and it all came back as I was becoming sober.

When I was like 11 I was scratching my face and a bit of loose fingernail fell into my eyelid and was constantly scratching at the inside of it. The pain was immense, lasted for about 30 minutes before I felt my eye twitch while mum was driving me to the hospital and the pain was gone.
i had a root canal when i was 10, but i was allergic to the numbing agent so it burned extremely so instead of numbing my mouth/gums, and apparently nobody believed me that i could feel anything, but they gave me another shot of the stuff just in case. i have a pretty huge fear of dentists to this day
When I was 14 I fell on a piece of rebar that went through my colon and all the way through my blatter after about $500,000 and a intestinal drain, catheter, and a colostomy bag (the fist two taken out while conscious) for 4 months I'd say that the drain was the worst
Forgot about the sepsis, blatter infection and testicular torsion from the fall

Getting my IUD in was stunningly horrible, and I had to have it done twice. They have to dilate your cervix to do it if you haven't had kids. Thing is, cervical dilation is one of the really painful parts of childbirth, but your body is sending hormonal signals to make it happen, which is kinda different from having it just forced open with a metal thing with minimal anesthetic. There was also this horrible feeling of internal scraping reverberating through my whole abdomen. Of course, it didn't last at that intense level for hours and hours like labor does, but it was still pretty bad.
Also, holy shit @Moist_master...

this reminds me of one time when i was playing with my uncles cats (even though im allergic, i love cats) and my eye suddenly started hurting way more than normal (a normal allergic reaction for me is that my neck gets super itchy). it ending up going like completely blind and it was super painful and while my sister was looking closely at it she saw a hair and went to get a Q tip to pull it out, and i could feel it like sliding along my eyeball, it was like a couple inches long. worst allergic reaction ive had and feeling that hair didnt hurt but it was fuckin weird

Wisdom tooth while coming out, was pushing my normal tooths root while my other wisdom tooth was coming out above. And after the wisdom teeth removal procedure. Let me mention, that procedure happened after 2 months of consistent pain, since our healthcare is amazing

When I was four, I touched a spiked, colorful, caterpillar.
It gave my hand a burning feeling and did not go away until my mom put some sort of powder and cream on it.

When I was ~7-8 I was shooting hoops in my backyard on a little plastic basketball hoop that was small enough for me to dunk on. One time I went to dunk and I lost my balance and I ended up face-planting into the concrete and shattering my two front teeth and forcing the cracked remnants of my teeth up into my gums. Parents rushed me to the dentists office, late at night and the dentist performed emergency surgery to remove the broken bits of teeth Thankfully they were my baby teeth so I'm fine now. My adult teeth took an extra year or so to come in because they were pushed up into my gums a bit from the force so I had a gap in my smile for quite a while
